Residential Program Manager

Education: Possess a BS or BA in a related area (social work, nursing, education, etc.) Prefer one-year experience supervising individuals with developmental disabilities (QIDP qualified).

The Opportunity

Ensures the operation of assigned caseload in accordance with agency mission and vision, Indiana Rules for Licensure, ICF/ID, and Medicaid Standards.

Essential Job Functions

1. Develops Person-Centered Individual Support Plans and conducts timely annual staffing for assigned consumers at locations and times of the consumer’s choice and/or convenience.

-Conducts annual Case Conference/Staffing and Person-Centered Planning; monitors monthly progress for consumers.

-Documents any updates or changes in the PC-ISP and ensures prompt implementation of such changes.

-Promotes the participation of consumers and their individuals of choice on an ongoing basis in discussion of plans and implementation.

2-Develops Risk Plans and meaningful Behavior Support Plans. Works with staff for implementation in a timely manner.

3-Completes General Risk Factors, Consumer Specific, Informed Consent and other priority assessments at least annually.

-Ensures that functional assessments are completed and reviewed annually.

-Ensures that consumers’ annual surveys are completed.

4 - Provides the training and ensures implementation of all PC-ISP’s to applicable people involved with the consumer.

-Conducts regular observations and tests staff to ensure competency in implementing plans.

5-Works closely with Residential Manager in facilitating day to day operation of group home. Includes, but not limited to, completing healthcare appointments and direct care as needed.

- Use of personal transportation may be required.

6-Works closely with Residential Manager to assure implementation of the Health Care Services Monitoring system for consumers well-being.

7 - Ensures consumers apply for and obtain all available benefits (SSI, Social Security, Medicaid, ABLE account etc.).

-Ensures continued compliance with all benefit requirements.

8 - Maintains consumers collateral information on the provided system in an easily accessible manner.

9 - Documents incident reports and submits to appropriate providers and agencies according to standard.

- Conducts other internal investigations including abuse/neglect, health/safety, fall scene investigations and injury of unknown origin.

- Ensure the recommended actions are followed.

10-Ensures residents have a safe environment.

-Free from abuse, neglect, exploitation and violation of rights.

-Medical needs are communicated to Health Services.

11-Maintains positive relationships and open communication with relevant individuals and organizations, including consumers individuals of choice.

12-Participates in Human Rights Committee meetings.

13 - Facilitates intake and exit discharge process and arrangements providing appropriate support services needed. Submit admission/exit summary to BDDS.

14-Participates in staff development, in-service training, and staff meetings as required.

15-Complies with agency Policies which include Employee Handbook and Code of Business Practices and Ethics.
